An unidentified risk is not a controlled risk. It is an incident waiting to happen.

Risk is everywhere: and that is normal
Risk management is not a separate discipline. It is a cross-cutting dimension of every industrial organisation. It runs across standards, functions and decision-making levels.
ISO 9001 introduced risk-based thinking as a structuring principle of the management system. EN 9100 requires formalised risk management at every stage of a programme. IATF 16949 includes FMEA and the control plan among its core tools. ISO 13485 requires product risk management throughout the life cycle.
Yet in many organisations, risk management remains a formal exercise: matrices filled in once a year, ratings disconnected from the shop floor, registers that change nothing in decision-making. The problem is not the principle. It is the way it is implemented.
From product to strategy: three levels of risk
Effective risk management brings three levels together into a coherent vision.
Product and process risks: this is the FMEA level. Analysis of failure modes, their effects and their criticality applies to the product, the manufacturing process and the production equipment. It feeds into the definition of key characteristics, control plans and preventive actions.
Operational risks: this is the level of the organisation’s processes. Supply chain, business continuity, competence, equipment, control of subcontractors. This is what ISO 9001 refers to when it requires the organisation to determine risks and opportunities to ensure the management system achieves its intended results.
Strategic risks: this is the level of the executive committee and the management review. Context of the organisation, regulatory changes, market, interested parties, investment decisions. These risks determine the long-term viability of the company and cannot be handled with a rating matrix.
An untreated product risk can become an operational risk. An unanticipated operational risk can become a strategic risk. The strength of effective risk management lies in linking these three levels, not in treating them in silos.
Our support
Consulting and structuring: maturity assessment of your risk management practices, structuring of the risk management process, integration into your quality management system. Definition of methodologies suited to your context and your standards.
Operational support: facilitation of FMEAs (product, process, equipment), definition of control plans, risk rating and prioritisation, implementation of control measures. Support for your teams on real projects.
Audit: assessment of the maturity of your risk management practices. Verification of consistency between the risk analysis and the control measures actually deployed in the field.
Training: transfer of skills to your teams so that risk analysis becomes autonomous, useful and alive.
Why QALIA?
Risk management fails when it is reduced to a rating exercise disconnected from the shop floor. Matrices completed in a meeting room, never tested against operational reality, produce risk registers that protect against nothing.
At QALIA, we anchor risk management in the reality of your organisation. Our consultants know the standards, but they also know the shop floor. They know that a documented risk is not a controlled risk, and that the real risk is not always the one listed in the register.
But some risks cannot be captured in an FMEA. Breakdown of cooperation between functions, gaps between what is prescribed and what is actually done, individual strategies that work around the system: when the issue is organisational, our Q-SYNQ® methodology takes over. By revealing the human dynamics and power plays that conventional tools miss, it identifies the risks your matrices cannot see, and builds responses that last.
